To Make an Appointment

All Dermatology-Allergology patients are seen by referral appointment only. We wish to make this process as easy as possible for you. This is how to make an appointment in 4 easy steps:

Step 1: GET REFERRED BY YOUR PRIMARY CARE VETERINARIAN. Please have you primary care veterinarian send in the general and dermatology referral forms.

Step 2: WE CALL YOU TO SET UP THE APPOINTMENT. Our receptionist will call you to schedule an appointment. The Dermatology-Allergology service is not open on the weekends or holidays.

Step 3: PREPARE FOR YOUR VISIT. You may make your visit more rapid and efficient by clicking on the link below, printing this form and filling it out in advance. Remember to bring the completed questionnaire with you to your appointment. Alternatively, the completed questionnaire can be mailed or faxed to us prior to your appointment.

If your pet is referred for skin testing, please note that certain medications may need to be discontinued prior to the appointment. Please refer to the “Your Pet and Skin Testing” brochure. Please call us if you have any questions regarding these important guidelines.

Step 4. DAY OF APPOINTMENT. We do require that the primary care-giver of the pet either be present at the appointment or be reachable by phone during the scheduled appointment time.

If your pet is referred for skin testing, bring your animal fasted. Since sedation is most often required, we ask you not to feed your pet at least 8 hours prior to your scheduled appointment. Drinking water is allowed.

Please note: The Dermatology-Allergology service is open from Monday to Friday except for statutory holidays.

When you are calling the Dermatology-Allergology service, please call our receptionist from Monday to Friday, from 8:00 a.m. to 8 p.m. so that we may best serve you and your pet. The receptionist will take your message. The dermatologist or the dermatology animal health technician will try to call you back as soon as possible.

Allergy Vaccines Refills
